School, Daycare, and Camp Forms

Forms take 3 – 5 business days to complete

We receive thousands of school, camp and sports forms every year. Our goal is to complete them efficiently accurately and get them back to you as soon as possible so that your child can participate in their activity without a holdup in paperwork. Please send your school, camp, or scout forms to us at least a week before they are due so that we have time to complete them.

Any form brought with to a well visit appointment will be completed at that visit at no charge.

A small fee will apply for forms sent in outside of the well visit appointment:

• School form/ Day Care forms: $10.00
• Boys Scout Forms: $20.00
• Camp forms are $20.00, with an additional fee of $10.00 per child per family, up to a maximum of $50.00 for patients with big families (if you do not present the form during the well child visit).

Forms can be faxed to us on 703-404-0286, or uploaded to our secure patient portal by following these steps:

• Log in to the Portal (it is best to use desktop browser instead of app)
• Select My Health
• Select Documents
• Select Add Document
• Type document name
• Type date
• Then choose a file to upload
• Open the file you want to send
• Click Save
• It will pop up stating: to confirm Add- select Yes
• Once it uploads, click on paper icon with magnifying glass under options tab
• Your document will pop up
• You will select Fax
• Type Who it’s going to
• Type in our fax number (703-404-0286)
• Select send
• Notification will pop up saying : “Your fax was sent”

The document will them come through our fax machine.
